Android create new form activity for input

i'm really newbie in android.

Now, i'm try to create some apps like "Money Lover".

I wanna create new form for input new data, so when i click button ADD in action bar, i want some form activity come up like bellow :

My Question is :

1. Is this use independent activity or use fragment.
2. How to call it from onClick button, is this same like startActivity(intent) ?
3. How to create layout that have Close button in top left and Save in top right like that picture.

Oya, that screenshoot from "Money Lover" apps from Play Store. I try to create some apps base on that apps.
